Hillendale Home Care begins with a FREE assessment
visit. During this visit, a Care Plan is written and reviewed
to ensure that structured and professional care is provided.
The client and family members assist with the Care Plan. This
free service usually takes an hour to complete and gives everyone
a clear path of duties and responsibilities.
The Care Plan is updated and modified as needed.
Caregivers are supervised regularly and client's satisfaction
is assessed to ensure the best possible service is delivered.
There is no long-term contract and service can be ended at any
time with no penalty. However, we do require a 24 hour cancellation
notice to ensure job continuity for our care givers.

relationship with us changes and/or additions are made to accommodate
client needs.

Hillendale Home Care is
committed to maximizing the independence and autonomy of our
clients and offer Geriatric Care Management (GCM) services
to clients who need more structured oversight of their day-to-day
living. Our GCM services start with a detailed assessment, which
addresses environmental, social, medical, psychosocial and any
financial needs. A Plan of Care addressing needs and offering
resources is then implemented.
Hillendale's Geriatric Care Management
Services will:
- Act as a liaison to families, overseeing care and promptly alerting
family members to problems.
- Assist with relocation of a client when care
needs change.
- Compassionately and expertly
assist in finding solutions to the ongoing challenges of aging.
